I work at the company that makes QQ (Tencent). If you want to learn more about QQ, please do visit my website. I created a page especially for foreigners about QQ: QQ signup help, I also list some of the QQ services.
I see a couple of misunderstandings in earlier replies. Please let me clarify:
1. I agree old versions of QQ (like 2005) were not very good (just my personal opinion). But now we have English Tencent Messenger 2008. This version rocks and future QQ versions based on the same platform will also rock. This version installs very cleanly on your OS.
2. Tencent uses virtual money so you can buy extra services. This makes QQ accounts also interesting for criminals though: if they can get your password, they can steal your virtual money. Tencent has long realized this problem and in the newest versions of QQ/TM they have a built-in virus/malware/worm scanner, there are also lots of other safety features. Links cannot be clicked directly etc. I think big steps forward and most advanced protection in the world. Sure, you still need to pick a good password, but I think it is very safe to use now.
3. QQ (or TM - Tencent Messenger) has indeed some amazing features. I agree with mirko that the screen capture tool is extremely useful. QQ is much more advanced than anything Yahoo/MSN/AOL have to offer. Unfortunately most extra services are (at the moment) Chinese-only, but we are working on changing that. You can of course still use the English TM or QQ without problems if you don‘t speak Chinese.
To answer your question: yes it is IM, yes you can send SMS with it (if you pay I think), but it is also much more. It‘s a whole lifestyle. It‘s part of Chinese culture.

I installed the 2005 english edition of QQ, don‘t use it often, for the threats reasons. Best feature, in my opinion, is the screen capture, click on a portion of the screen and paste on messenger window, the snipping tool of Window is far beyond.

My daughter loves it but it makes a huge mess of her operating system... (so i dislike it)
I won‘t allow her to use my computer to even access qq.
